Appointments without pt records

Does anyone else recognise this problem? What should we do about it?

If you need emergency care outside of your own area, then there will be no records available, this means that the staff which are dealing with you have to use the skills they alledgedly have, rather than just read what someone else has stated. This can be a good thing, as long as the person concerned is capable of doing the job.

A great deal of the record is made up of what the patient tells you anyway! Many patients know better than the doctor their history and probably better than the notes. What can be dangerous about an outpatient appt taking place without notes? Unless you intend to give results without knowing them! If they were going to prescribe medication it's unlikely that the patient or GP cant tell you the current drugs and the notes cant really be relied on as an up-to-date record of current medication. I wonder what they think the dangers are.

Having witnessed the crap service my father received from 4 different doctors over the same problem, I dread to think how much worse it would have been if they were working without notes.

I agree mad jock. The paper records should be detailing complex histories, investigations/results, treatment plans etc. I know the patient's story is important but the written record is important too. I wonder if the electronic health record is the answer to the problem but I know patients who say they will opt out of it because they are worried their data will be lost like the child benefit data loss. They can't see that opting out will add clinical risk for them.

By virtue of the fact it's a hospital appointment means you are seeking specialist care, maybe you are seeing several specialists ,and one set of tests or treatment may have a bearing on another. But what if patient doesn't know or can't remember what was said before, is confused or has difficulty hearing or communicating surely then records do help.

records are essential, you can't expect a patient to know the results of their last 5 blood tests. They also ensure continuity of care, I think electronic records will be a step in the right direction.
