Composition points at photographer leaving enough space for the effects when shot was taken.......also a couple of bits do kind of look 'Shopped'
So I'll go with 'Faked'

I'd say that's a pen torch, probably with a blue tint. The exposure time (30 seconds) is more than enough to allow that effect to be created without showing the arm movement.
